A Michigan company has been fined $2,800 by OSHA after fatal accident in July involving two of its employees.
On July 17, 2012, Dennis Gougeon, 47, and Dustin Dean Hale, 29, were spraying high pressured water on a fly-ash bin at Packaging Corporation of America in Tomahawk. The ash ignited, seriously burning the two men. Gougeon died from his injuries a day after the incident. Hale died at a Minnesota burn center on July 28.
Both of the men were working as contractors from Inland Systems of Ontonagon, Michigan.
Previously, Inland Systems had no record with OSHA.
