Milwaukee County Executive Scott Walker says nearly all the concrete panels on a parking structure were not installed according to plan.
An independent firm started investigating after a 13-ton facade slab fell off the structure, killing a 15-year-old boy and injuring two others in Milwaukee June 24.
Walker says only four of the 296 panels were installed according to original plans and the county has found no authorization for the change.
The firm did not investigate why the slab fell. The district attorney and sheriff are investigating that.
Walker also recommended the county take off the facades, which would cost about $5.4 million. The county board has to sign off on the decision, which would keep the structure open.
The county is considering legal action to recoup costs.
